*We are welcoming of all Patients. If for any reason a family decides to leave the practice, your child's account will be made inactive. Upon reentry for any appointment, the parent or caregiver will be asked to fill out new demographics to become an active patient. In addition, our office will require a release of medical information from the prior pediatrician for continuity of medical care. These steps are taken to ensure a smooth transition of care and to maintain accurate medical records. We sincerely appreciate your support.
